Care - home assessmentSocial care services may need to carry out a home assessment as part of an assessment of your needs. For example, you may be struggling with daily living, such as getting out of the shower, if you have returned from a stay in hospital. A member of the social care assessment staff or a health worker may need to carry out a home assessment to establish where the difficulties lie.
It could be that before your discharge, you and the social care assessment staff or occupational therapist will visit your home to check out any problems with equipment, alterations or further rehabilitation you may need. More often you will not have needed to be in hospital but will be experiencing some difficulties managing at home. Our staff will then visit you at home to discuss what might help you.
