Tier two is a multi-component service that encourages behavioural change and incorporates healthy eating and physical activity components.

It operates as a twelve week course run as a series of group sessions, and is funded by North Yorkshire County Council.

Patients matching the following criteria are eligible for the tier 2 12 week multi-component group programme:

  • Aged 18 or over*
  • Have a BMI >25 without comorbidities or managed comorbidities
  • Living in North Yorkshire

However, patients in the following categories are not suitable / eligible for referral to tier 2 services:

  • Pregnant, post-natal prior to attending post-natal check, or breastfeeding
  • Have a BMI >35 with significant, unmanaged co-morbidities
  • Have had bariatric surgery in the last two years**

* Public health has commissioned a community weight management service for children aged five to 19 above the 91st centile and a residential weight management service for children aged eight to 17 above the 99.6th centile.

** Current guidance states that these patients must be followed up by their bariatric team for the first two years post-operation as there may be risks to the patient regarding malabsorption and nutrition.

For those not eligible for tier 2 services, advice can be provided regarding appropriate community-based activities (tier 1 intervention).

Tier two programmes offered in each district of North Yorkshire

The tier 2 lifestyle weight management service for Craven is delivered by Craven District Council and funded by North Yorkshire County Council public health.

The healthy lifestyles programme provides a free 12 week multi-component weight loss programme. Patients must be referred by a medical professional using the appropriate referral form.

The tier 2 services for Hambleton ('Take That Step') are run by the Take That Step team, supported by Hambleton District Council, and funded by North Yorkshire County Council.

Hambleton, Richmondshire and Whitby clinical commissioning group provide additional funding for clients with a BMI <25 for the additional physical activity element of the programme, i.e. musculoskeletal or mental health conditions.

Health professionals must register with the scheme using the registration form below. Once a registration form has been received an electronic version of the referral form will be sent to the health professional, which can then be used immediately.

The Harrogate 'Fit 4 life' tier 2 weight management service is run as part of Harrogate Active Health. It is a 12 week programme offered at venues across Harrogate district that aims to promote healthy eating and physical activity.

The programme consists of group sessions involving individual weigh-ins, physical activity and weight management advice. Before participating in group sessions, each individual must attend an induction with the instructor (see advice leaflet below).

The Harrogate Active Health scheme offers a selection of 12 week exercise courses (see information documents below), some of which are funded by the patient and some of which are funded by other organisations.

The tier 2 lifestyle weight management service for Richmondshire is run by Richmondshire District Council and funded by North Yorkshire County Council public health. The service is an adult lifestyle service supporting overweight and obese adults to lose weight and maintain a healthy weight and healthy lifestyle.

This physical activity service supports adults with low to moderate medical conditions, post physiotherapy and / or a history of sedentary lifestyle to improve their participation in physical activity and maintain their physical health.

The Selby District Council tier 2 programme, 'Move it Lose it' is managed and delivered by Wigan Leisure Culture Trust.

Wigan Leisure Culture Trust deliver the physical activity element of the programme, while Slimming World delivers the nutrition aspect of the programme. The programme is run as a 12 week rolling programme, where participants attend Slimming World classes, followed by a choice of physical activity sessions run by Wigan Leisure Culture Trust.
