We aim to treat everyone who uses our service in a respectful, sensitive and professional manner. If you are unhappy with the service you have received from us, please talk to the worker or to their manager, who will try to resolve things for you. If you are not satisfied with this response you can submit a complaint.

'Your voice' questionnaire
We would like you to take part in our 'your voice' questionnaire. This will help us understand how you feel about your life, so that we can help you as much as we can.
The questionnaire takes about 30 minutes. You must finish it all in one go and click ‘submit’. If you close the page before the end, it won’t save anything and you will have to start again.
We will keep your answers on a secure computer system, where only trusted staff can see them. We will not tell anyone what you have said unless it is really serious and urgent, like someone being in danger. If you would like to have a copy of your questionnaire, you can ask your worker to arrange that.
