Everyone is at risk of developing heart disease, stroke, type 2 diabetes or kidney disease. These diseases can often be prevented and an NHS Health Check can help by assessing the risk and giving personalised advice on how to reduce it.

NHS Health Check invitation and offer

A small change to the invitation letter can increase uptake of the NHS Health Check.

New results from a research trial show that making small changes to the NHS Health Check invitation letter can increase uptake at no extra cost. A Department of Health research team worked with Medway Council to rewrite the invitation letter and robustly test its impact. This resulted in a 13 per cent increase in uptake (four percentage points). Please use these letter templates in the future:

The risk assessment

All practices are recommended to use standardised templates to record NHS Health Check risk assessment information. Standardised templates for both SystmOne and 5 byte clinical systems can be uploaded directly on practice systems.

NHS Health Check Results card

The alcohol section of the  NHS Health Check results booklet (pdf / 732 KB) has been updated to reflect the UK Chief Medical Officers' low risk drinking guidelines. A copy of the results card should be given to patients after their NHS Health Check.

Annual review for those at high risk

Our move more - exercise and fitness and healthy weight and eating well pages support health professionals to enable adult patients who are overweight or obese to select appropriate physical activity or weight management services in North Yorkshire and York.
