We provide an intense, short-term support service for people who have contacted us for the first time, seeking social care support.
Will talk to you to find out what your needs are and if you are eligible, you will be offered up to six weeks of support to help you quickly get back in control of your life. For example, you could be offered equipment to help you do things independently.
If you need longer term support than this service provides, you will need to complete an assessment of your social care needs.
