We provide an intense, short-term support service for people who have contacted us for the first time, seeking social care support.

Will talk to you to find out what your needs are and if you are eligible, you will be offered up to six weeks of support to help you quickly get back in control of your life. For example, you could be offered equipment to help you do things independently.

If you need longer term support than this service provides, you will need to complete an assessment of your social care needs.

We have created a series of videos to help explain the short-term assessment and reablement process. They also feature people who have been through the reablement process talking about their experiences.

This video explains the short-term assessment and reablement process.

Jim

Jim needed a carefully planned reablement programme to retain his independence and achieve what the family wanted, after a stay in hospital.

John

For John, receiving the reablement service after several months in hospital meant a chance to resume active life on the family farm.

Peggy

A severe stroke meant that when Peggy returned home from hospital, she struggled with everyday tasks. With a carefully planned programme of reablement, she regained her independence.
