Rockwood Score 5 - Mildly frail people have more evident slowing and need help in higher order including independent activities of daily living for example, finances, transport and medications.

Type of wider community activity or support likely to be relevant

  • organisations providing support in the home; medicine promoting; alerting GP as needed (able to support digital contact with GP)
  • organisations that provide support to appointments.  Community transport may be appropriate dependent on the conditions
  • volunteer support to enable individuals or families to access ‘Ask Sara' for relevant advice including sourcing appropriate equipment
  • lunch clubs where individuals can socialise
  • may need welfare checks undertaken by trained staff
  • condition specific advice, support or possibly exercise classes
  • additional support at pre physical class level (exercise at home programme) to enable individuals to remain mobile

Need to differentiate between those needing more support to access activities and those that will need assistance with personal care while attending an activity

  • Community Transport is a not for profit transport provision, run by the community for the community. Information on local providers can be found on Go Local website
  • Home from Hospital on the carers' resource website – North Yorkshire is a free service for people aged 18 and over who are being discharged home and need extra support
  • our Ask Sara service provides impartial information and advice about equipment to make daily living easier
  • blue badges help disabled people with severe mobility problems to access goods and services by allowing them to park close to their destination. Even if someone no longer drives, they can obtain a blue badge to use if a family member, friend, neighbour or volunteer drives them to access services or shops
  • our living well coordinators work with people impacted by a change in circumstances or facing loneliness and isolation. Coordinators help people find ways to improve their situation perhaps by increasing their participation in local activities and/ or helping them gain confidence and appropriate levels of self-determination and independence
  • the warm and well website in North Yorkshire offers practical solutions to reduce fuel poverty and supports people to stay warm and well in their homes
  • many people are entitled to a pension top up, even if they have a private pension and savings, more information is available on the pension credit eligibility checker on the government website
  • online home fire safety check on the Safelincs website
  • our NY Connect directory is the free community directory for North Yorkshire, providing information about a wide range of local community groups and services
  • North Yorkshire Sport, Strong and Steady programme is designed to encourage people to become more active and reduce the risk of trips and falls
  • information about North Yorkshire libraries
  • help for an individual page on the AbilityNet website provides information regarding digital skills from trained Digital Champions
  • supporting people affected by dementia across North Yorkshire webpage on the Dementia Forward website
  • Social care can assess people over 18 who have, or appear to have, an illness or a disability and may benefit from receiving care services
  • our care services list contains information about social care services available in North Yorkshire and lists care providers and nursing homes on the approved provider list (APL)
