Types of wider community activity or support likely to be relevant

Ongoing as appropriate:

  • specialist one to one work to support the individual
  • hospice support
  • death cafés. opportunities to explore how the individual wants to live for remaining time 
  • important to maintain pre-existing relationships with volunteers and to sustain previous connections
  • specialist physical activities offered at a personalised level to promote mental wellbeing

Support for family and carer

  • Register as a priority customer with utility companies. This means, should an incident occur which does trigger a major response, utility companies have the information they need to prioritise help. Links to main utility providers are:

  • our care directory contains information about social care services available in North Yorkshire and lists care providers and nursing homes on the approved provider list (APL)

  • Social care can assess people over 18 who have, or appear to have, an illness or a disability and may benefit from receiving care services
