Rockwood Score 4 - while not dependent on others for help, often symptoms limit activities. Complain of being ‘slowed up,’ or being tired during the day.

Type of wider community activity/support relevant

As Rockwood Score 3 plus:

  • community transport
  • organisations providing assisted shopping and befriending support
  • welfare checks with specific focus may be needed
  • condition specific exercise classes, for example ‘Exercise after Stroke Class’
  • some community based physical activities would be suitable with guidance from GP

Community groups or organisations that provide this support

  • organisations with which the individual already has a relationship may continue to offer support.
  • small to medium sized interest based or geographically based voluntary or community sector organisations with appropriately trained staff or volunteers

  • older people of pensionable age are entitled to a bus pass giving free off-peak travel on local bus services anywhere in England
  • our Ask Sara service provides impartial information and advice about equipment to make daily living easier
  • blue badges help disabled people with severe mobility problems to access goods and services by allowing them to park close to their destination. Even if someone no longer drives, they can obtain a blue badge to use if a family member, friend, neighbour or volunteer drives them to access services or shops
  • our living well coordinators work with people impacted by a change in circumstances or facing loneliness and isolation. Coordinators help people find ways to improve their situation perhaps by increasing their participation in local activities and/ or helping them gain confidence and appropriate levels of self-determination and independence
  • the warm and well website in North Yorkshire offers practical solutions to reduce fuel poverty and supports people to stay warm and well in their homes
  • many people are entitled to a pension top up, even if they have a private pension and savings, more information is available on the pension credit eligibility checker on the government website
  • online home fire safety check on the Safelincs website
  • our NY Connect directory is the free community directory for North Yorkshire, providing information about a wide range of local community groups and services
  • our living well smokefree service has a team of advisors with lots of experience in helping people to stop smoking for good
  • The adult weight management service is a 24-week programme designed to improve physical and mental health by helping participants eat well and get moving
  • information about North Yorkshire libraries
  • our adult education and training courses
  • volunteering in North Yorkshire on the Community First website
  • local community Facebook groups, community noticeboards and friends, family and neighbours are also good sources of information.
