Being inactive can lead to serious health problems like type 2 diabetes, heart disease, high blood pressure, stroke, obesity and even some cancers. That is why we offer Fit 4 Life, a 12-week exercise referral programme designed to help people become more active and improve their health.
How you can take part
Your health professional can refer you if you are inactive or have certain health conditions. For some people, this can even be an alternative to medication and may reduce the need for frequent GP visits.
The safe, supervised exercise plan is a short-term programme with long-term goals – helping you build confidence and make physical activity a regular part of your life.
You may be eligible if you:
- are aged 16 or over
- do less than 30 minutes of activity a week
- are at low to moderate health risk
- would benefit from being more active
- are motivated to take part in gym or swim-based exercise
We welcome referrals for people with these health conditions:
- overweight or obesity
- controlled high blood pressure
- mild to moderate mental health issues, for example, stress, anxiety, depression
- controlled diabetes (Type 1 or 2)
- arthritis or osteoporosis
- asthma or chronic obstructive pulmonary disease
- musculoskeletal issues, for example, post-physio for knee, hip or shoulder
- family history of heart disease
- physical disabilities
If you are unsure whether you qualify, ask your health professional to contact us to discuss your needs. We accept referrals from health professionals who:
- know your medical history and current health
- can judge if the programme is suitable for you
- can provide basic health details , such as your blood pressure, weight, contact information
- referrers must also inform your GP using an agreed process
The programme is not suitable for high-risk individuals, such as if you have an unstable heart condition, severe mental health issues or late-stage pregnancy.
You can only be referred once a year.
