Rockwood Score 6 - People need help with all outside activities and housekeeping. Inside they need bathing and may need minimal assistance (prompting) with dressing.

The person may need support to get out and about or to leave the home. The individual cannot socialise independently and needs help with household tasks and prompting to eat and drink and to take medication correctly, as well as prompting to wash and dress. This is non-regulated support below the threshold for personal care. Appropriately trained employees or volunteers could deliver this support. Consideration of whether the person has capacity to manage their finances must take place before referral to support organisations.

Types of wider community activity or support likely to be relevant

  • meals on wheels service delivery – may need help with heating up meals
  • meal preparation
  • reminders to complete personal tasks and take medication
  • shopping completed

Social needs of the individual may be met by volunteers:

  • befriending in person or by phone
  • transporting to social activities in community-based provision. This provision should have skilled staff to support needs of attendees
  • support organisations offering one to one support with specialist advisors either online, by phone or in person for friends, family and the individual
  • exercise at home programmes as part of the wider support offer to the person

Support for carers is important at this stage:

  • peer support
  • carers sitting service and / or respite day service support
  • supported connections with wider community

There is a need to keep all those supporting the individual in the loop.

  • organisations with which the individual already has a relationship may continue to offer support
  • small to medium sized interest based or geographically based voluntary or community sector organisations with appropriately trained staff or volunteers
  • volunteer led activity or interest groups with appropriate knowledge, skills and training
  • social enterprises with trained volunteers and/or paid staff
  • privately provided activity groups
  • private providers
