We are implementing our care and support Community Hub provision improving local social care services, enabling the residents of North Yorkshire to thrive, especially those recovering from a long hospital stay and supporting people diagnosed with dementia. We are on a mission to reduce long hospital stays and help people get back to living independently in a safe and supported way.

We are extending our well established and successful extra care provision, championing people’s independence and further enabling people to remain in their communities, with care and support on their doorstep. With an increasingly diverse range of needs, our executive has approved a new 10-year programme which would see more Extra Care facilities being built alongside similar services for younger adults who may need care and support.

We are launching an intensive care service, uniquely to support people with mental health needs access intensive, personalised support, at a critical time for them.

We are looking to recruit to a variety of permanent roles across a variety of locations within North Yorkshire.

All of our roles allow a flexible approach to working. While virtual working is promoted as a modern and efficient way of delivering services, the need for face-to-face visits and office working will also be required, depending on the needs and circumstances.

Social Worker opportunities

It is an exciting time to join our social work teams. We have social worker opportunities in the following four services:

Mental health

North Yorkshire Council is seeking Mental Health Social Workers to join their Adult Community Mental Health Team. This role involves supporting adults with mental health needs through a recovery-focused approach, fulfilling Care Act responsibilities and safeguarding duties. Applicants must be qualified and registered social workers, with mental health experience preferred.

This role involves working collaboratively with multidisciplinary teams, including Tees Esk and Wear Valley NHS, to support individuals on inpatient psychiatric wards. You will promote mental health social care services, apply the recovery model and conduct Care Act assessments in a person-centred way. Responsibilities include safeguarding, mental capacity assessments, complex case management and potentially training as an Approved Mental Health Professional. The role welcomes newly qualified social workers and offers supervision, training and development opportunities.

Intermediate care

Our Intermediate Care Team takes pride in delivering short-term, high-impact support to help people recover and stay independent. You will work in people’s homes and care settings to prevent hospital admissions, support safe discharges and create personalised care plans. Collaborating across health, social care and voluntary sectors, you’ll play a key role in integrated care and safeguarding.

Prevention and access

The Prevention and Access Team supports individuals at their first contact with adult social care by providing professional advice, strength-based conversations and timely access to equipment and adaptations. They assist with direct payments, carer support, safeguarding concerns and connecting people to community resources. The team also conducts proportionate Care Act assessments, supports access to employment and provides sensory equipment and guidance, including managing the Certificate of Visual Impairment Register.

Adults and older adults

We have a variety of exciting roles available in our local Adult Social Care Services. We offer a range of support options to help individuals with health conditions or disabilities maintain independence for as long as possible. These include extra care housing, supported housing, residential care homes, community-based support and day centres. Extra care housing is ideal for those whose needs make standard housing unsuitable, providing independent living with access to care. Residential care homes offer accommodation, personal care such as bathing and dressing, mobility assistance and social activities. Day centres and community support services cater to people with higher dependency needs, offering tailored support and a range of activities including employment programmes, gardening, cookery, arts and crafts and fitness.

The Occupational Therapist will assess and support adults with physical, sensory, and mental health conditions, helping them achieve person-centred goals and improve their independence and quality of life. The therapist will collaborate closely with health professionals, community teams, voluntary and independent sectors, and public health colleagues to deliver integrated care and empower individuals to remain in their communities.
